We collaborate with Residential Care Communities that invest in ensuring the right residents are matched to their homes. Communities pay us (up to 100% of the first month’s fee) to:
- Pre-screen prospective residents for care needs and compatibility
- Ensure families touring their community are well-informed and serious candidates
- Reduce move-outs and improve long-term resident satisfaction
This placement option is not available for individuals who are currently on Medicaid or have already been approved to use Medicaid as their payment source for residential care. If you fall into this category, please visit options #2 and #3 down below.

- We will notify local Medicaid-accepting providers about your care needs—they may contact you directly

Medicaid Placement Navigation — Fee Based Services

Navigating Medicaid-funded care placement can be confusing and overwhelming. Our Medicaid Placement Navigation Package is designed to guide families step-by-step for a one-time fee, paid by the family and/or an advocate.
This service includes:- A full assessment of care needs and financial considerations
- A comprehensive, unbiased list of Medicaid-approved care providers
- Education on what to look for and how to prepare for tours
- Coordination of tours and preparation of materials
- Assistance with documentation and move-in planning
No Referral Fees from Medicaid-Participating Communities. We do not accept referral fees, commissions, or financial incentives from any communities or providers that serve Medicaid-funded residents. This ensures that every recommendation we make is based solely on the needs and best interests of the individual not influenced by provider payments.

Travel Companion & Relocation – Fee Based Services
(Inquire for a quote based on travel distance, care needs, and staffing selection)
For families and/or advocates coordinating placements from out of state—or for clients needing extra support during relocation—our Travel Companion Package provides:
- Full placement search and navigation services for families and residents located outside Washington State
- Coordination of all virtual or in-person tours and meetings
- Development of a relocation plan, including health considerations, packing guidance, and transportation coordination
- Pre-arrival assistance, including helping to set up the new living space if needed (furniture delivery, essential supplies, comfort items)
- A dedicated CBP Travel Companion who will travel with the resident from their current location to their new home in Washington State, ensuring safety, comfort, and emotional reassurance throughout the journey
- Post-arrival assistance with settling into the home and follow-up check-ins to ensure adjustment
You don’t have to live out of state to need relocation support.
Some local families face similar challenges when moving a loved one into a new residential care setting. We can also assist with:
- Coordinating trusted movers and setting up logistics
- Having someone present to receive furniture or deliveries
- Preparing and setting up the resident’s living space before they arrive
- Reviewing contractual paperwork and discussing possible issues in advance
You can choose the type of travel companion staff to accompany your loved one. During your consultation, we’ll help you understand the benefits of each option and ensure the care provided during relocation aligns with the individual’s medical and emotional needs.
- Optional Companion Levels (additional caregiver fees apply):
- Standard CBP Companion – For basic supervision and emotional support
- Licensed Practical Nurse (LPN) – For moderate medical needs and mobility support
- Registered Nurse (RN) – For advanced medical monitoring during travel
- Licensed Social Worker (LSW) – Ideal for clients with memory care or behavioral health needs
- Based on the number of caregivers required
- Depending on the level of licensure or specialized support selected
- Passport, visa, and documentation fees (for international travel)
- Medical clearance or special transport accommodations, if required

Note: Travel companion services are not available in all countries. Availability depends on visa regulations, safety conditions, and regional accessibility. We will confirm eligibility during your consultation.
DisclaimerMedicaid-funded residents may use our relocation services if their case manager has formally denied state-funded relocation support. In these cases, the full cost of the relocation service must be covered by a family member, legal representative, or advocate. We are not permitted to accept direct payment from Medicaid-funded residents, in accordance with Washington State law.

Not every individual is ready for—or appropriate for—licensed care settings. Some may thrive in independent apartments with supportive services, or need help transitioning from one type of housing to another. That’s where our Housing Navigation Service steps in.
We assist with:- Housing searches for individuals who need apartment living, senior housing, or subsidized housing programs with varying levels of accessibility and support
- Assessment of care needs to determine whether independent, assisted, or memory care housing is most appropriate
- Guidance through rental eligibility requirements, documentation, and background checks
- Connecting clients with community-based support services, such as Medicaid case management, home care agencies, visiting nurses, or behavioral health support
- Waitlist navigation and support in completing housing applications
- Tenant education, helping individuals and their advocates understand lease agreements, rights, and responsibilities
- Pre-move preparation, including coordinating trusted movers and setting up logistics
- In-unit setup, including furniture delivery, basic home goods, and accessibility adjustments if needed
- In-person or virtual tours of potential apartment or housing options
- Post-move follow-ups to ensure smooth transitions and long-term sustainability
- Clients transitioning out of family homes or facilities but who still need oversight and soft support
- Adults with disabilities, behavioral health diagnoses, or chronic illnesses who can live semi-independently
- Advocates, friends, or case managers supporting someone who doesn’t fit into traditional long-term care models
